Enhancing Communication to Optimize Care Coordination teaches HCPs to effectively (1) identify patients who need comprehensive care coordination to bridge the gap between hospital care and outpatient services and prevent rehospitalization, (2) communicate during patient transitions in an accurate, timely, and standardized way so the receiving clinician has the information needed to deliver appropriate patient care, and (3) engage in shared decision making and empathetic communication with patients, family members, and caregivers when assessing transition choices and related concerns.
