Care coordination is essential to help prevent adverse events, improve efficacy, and help healthcare professionals (HCPs) ensure that care is patient centered. Lack of appropriate care coordination can result in medical error and even patient death. This course provides emergency clinicians with the knowledge to identify patients with the greatest need for enhanced care coordination and discusses best practices in communicating with other HCPs to improve care coordination.
