Given the complexities and challenges faced by healthcare professionals (HCPs), clinicians must rely on effective and standardized tools for accurate and timely delivery and transfer of patient information. Closing the loop, a frequently used term in the healthcare industry, is an effective communication model designed to ensure that HCPs exchange vital information during patient care, handoffs, and care transitions. Closed-loop communication has been shown to improve team collaboration and minimize medical errors. The goal of this course is to identify strategies for improving communication by closing the loop (including the use of SBAR and the teach-back method).
